Yep its fixed when seo is turned off
That is a problem related to the hack you are using.
Search for $vboptions[clearpngurl] within editor_toolbar_on and replace with $vboptions[cleargifurl]

Okay thanks. I just that to my hidden skin and the problem is still there. Cache is cleared. Works fine with vbseo off so it might be that ?
Please check vBulletin Settings >> Admincp >> Style & Language Settings >> Location of clear.gif and make sure you have www.domain.com/clear.gif over there

Update: I've changed "clearpngurl" to "cleargifurl" in showthread_quickreply template and that resolved the issue.
