...My question is, does it MEAN anything? I fear that my site has been penalized by google and I am not sure why.

It's worth having a look at what it returns but I certainly wouldn't put a whole lot of faith in the results. For example, they flagged register.php as a bad neighborhood link.
On the other hand, it did find a 404 for me for my main forum index.
It also flagged a number of thread titles (mental health forum) as bad neighborhood links.
All in all, like most of these things, think of it as a cute toy without a whole lote of substantive value.
