Did you make any changes to it through cPanel/Plesk/anything else?

If you look at the file's properties, or your web server logs, when was it last modified?

(.htaccess files cannot change their contents spontaneously. Someone has changed it. Is it possible that you don't know all the ways it could be changed?)

You could, and it would probably fix the issues, but if you made those changes for a reason, replacing them might cause issues later.

Honestly, because I can't understand how or why the changes were made to htaccess, I couldn't be 100% certain.

If you are fairly certain that you didn't mean to make the changes, then upload the .htaccess that came with the vBSEO package (remembering to alter it to reflect your domain if you are using the non-www to www redirect).

You should ask your host. Some hosts add stuff like that to htaccess.
