Our website uses vBulletin to post news and articles to the main site. A regular article will get around 30-70 linkbacks, but our popular articles can get between 100-200 linkbacks. A few rare ones even go above 250 linkbacks.
When we were still on our old server (single core Xeon) during peak times, the server's CPU usage averages 80-90% after we release a popular article. We often had to disable Linkbacks for a day or two when a popular article is released.
We have since upgraded to a faster server, and performance has been much better. However, we still think that vBSEO linkbacks could still use some improvements.
I recommend having a temporary/separate table just to record all incoming linkbacks (even before moderation). Then have a scheduled task to sort them out (count the total hits, do filtering/blacklist check, etc).
Probably not a lot of websites will have similar cases, but it would be nice if something like this can be implemented.
